Every instruction is different and this ‘Information Form’ is not intended to be legal advice. It is an overview only. We will contact you after you have completed this form to obtain further information which will assist our office in the preparation of your will.

The information you have provided to us on this instruction sheet is necessary for the preparation of a Will and will be kept as a confidential record of your instructions by our firm.

Note that this form itself does not constitute a Will.

4. Appointing Your Executor

The Executor is the person you appoint in your Will to take care of your Estate and ensure that your wishes are adhered to. The Executor must be over 18 years of age and be someone that you trust. You may appoint more than one executor, but we usually recommend one person and then a substitute.

If your first nominated executor cannot act, your substitute executor will act in their place.

The Executor should not be a foreign resident, where possible.

5. Naming Your Beneficiaries - Specific Gifts

Do you want to leave any specific gifts in your Will (e.g. jewellery to x person, property to x person)?

If not, then leave this section blank and move to Section 6.

For each additional gift be sure to specify the gift, the full name of the beneficiary, and the address of the beneficiary.

7. Additional Considerations

Have you completed a binding death benefit nomination with your superannuation fund?
This ensures that your super funds are distributed according to your wishes. Each super fund is different, so you may need to renew your binding death benefit nomination every three years.

Do you currently have a Power of Attorney or Enduring Guardianship document in place?
